Direct Primary Care Membership

Canopy’s Direct Primary Care service offers all aspects of primary care for a simple monthly fee with no co-pays or surprise billing.

Your monthly membership provides:

  • 60-minute initial visits to review your past medical history as well as discuss current health needs and goals

  • 60-minute wellness exams, including complete physical exam

  • Same-day and next day appointments for urgent concerns

  • Telehealth appointments by phone or video

  • Timely direct access to your provider after hours for urgent issues

  • Access to zoom and in person wellness classes

  • Home visits (an additional fee may be required for patients outside the Great Barrington area)

Membership Rates

Ages 18-39
$140 per month

$260 per month

Family Members of Canopy patients receive a 25% discount off their membership fee.

Important to know:

  • There is a one-time registration fee of $75 required for new patients (waived for former patients).

  • There is a three-month minimum enrollment for all primary care patients. After 3 months, you may cancel at any time with a 30-day notice.

  • If you decide to rejoin later, a re-enrollment fee of $200 will be applicable.
